ABOUT THE ROLE

Transitional Care Program provides a low-intensity, time limited, and coordinated interprofessional restorative model of care to adult clients who have experienced a recent decline in independent function. The program strives to support clients in returning home, either independently or with appropriate community-based resources.

Reporting to the Clinical Manager – Palliative & Transitional Care Services, the Social Worker provides services to clients and/or their Substitute Decision Maker (SDM), family members, and other support persons in the client’s life when applicable. The Social Worker is an integral part of the interprofessional team, providing a social work perspective and acting as a liaison and/or consultant to internal and external programs within the scope of practice.

The goals of Social Work practice are to restore, maintain, and enhance the quality of life by mobilizing strengths, supporting coping capacities, assessing and supporting people to modify patterns of relating and acting, linking people to necessary resources, addressing environmental stressors individually/ collectively, and providing psychosocial education related to wellness and subjective well-being.
