ABOUT THE ROLE

Reporting to the Clinical Manager – Diabetes Health & Wound Services, the Social Worker is a part of the Regional Wound Care Program, which operates within the broader Wound Services Program.  The Social Worker provides services to clients and/or their Substitute Decision Maker (SDM), family members, and other support persons in the client’s life when applicable. From herein, the job description will refer to clients and/or others for this purpose.

The Social Worker is responsible for providing comprehensive assessment and intervention that addresses the psychosocial needs of the clients and/or others in the Wound Program. The Social Worker is an integral part of the interprofessional team, providing a social work perspective and acting as a liaison and/or consultant to internal and external programs within the scope of practice.

The Regional Wound Care Program (Wound Services) is aimed at reinforcement and integrating best practices in wound care throughout the Northwest. This involves taking a specialized inter-professional approach to wound care by providing education, support and consultation as needed for complex and difficult wounds.
