ABOUT THE ROLE

Reporting to the Manager – Regional Rehabilitative Care Program (RRCP), the Stroke Navigator will provide in person and virtual navigational support for clients with stroke and their family, for Thunder Bay, the region, and First Nations. Services will ideally start at Thunder Bay Regional Health Sciences Centre (TBRHSC) or St. Joseph’s Care Group (SJCG) and continue to follow the client in their own community.

The Stroke Navigator will work as part of the Community-based Stroke Outreach team. Embedding navigation into Community Stroke Rehabilitation is essential to support persons with stroke to reintegrate into their communities. This includes navigational support that connects, supports, educates, and empowers individuals to reach their goals and return to meaningful life activities and roles within their homes and communities. The Stroke Navigator will act as a trusted point of contact, offering personalized guidance, education, and improved access to coordination of healthcare services, as well as other needed resources. This is a pilot program and as such the Navigator will participate in evaluation and recommendations for the program going forward.
