ABOUT THE ROLE

Reporting to the Director - Regional Programs, the Manager provides leadership to improve access transitions promote adoption of best practices and system monitoring and evaluation for rehabilitation care in North West Ontario that advances the regional and provincial priorities consistent with the mission vision and values of St. Joseph's Care Group (SJCG). The Manager must be able to lead self engage others achieve results develop coalitions and conduct systems transformation to support rehabilitation across the Northwest. The Manager uses a collaborative approach inclusive of people with lived experience and family partners to plan budgets and service delivery manage fiscal and human resources and achieve performance targets for quality client outcomes.

Accountable to SJCG leadership and guided by Ontario Health North West priorities the Manager plays a key role in building relationships with local regional and provincial health care partners to advance integration of rehabilitation service delivery ensuring access across the continuum of care and smooth transitions between levels of care. Working within the North West region health structure the manager collaborates with the four Ontario Health Teams (OHT) and non-signatory First Nations communities/agencies including Federal Indigenous agencies (i.e. ISC in local system planning) and the Regional Specialized Services Network (RSSN) in regional planning for specialized rehabilitation service. The Manager facilitates an environment of collaboration and cooperation with leaders physicians staff partners and clients and families demonstrating effective communication and engagement to improve the access to and quality of rehabilitation care received by residents in the North West.
