Position Title: Full-Time Direct Support Professional - Adult Supported Group Living Services - Multiple Locations

Compensation: $ 25.78- $28.43

Nature of Position:

We are seeking dedicated and compassionate Direct Support Professionals to join our team on a full-time, permanent basis. The successful candidate(s) will be tasked with providing a variety of personalized supports for individuals who currently reside in our Adult Supported Group Living Services. This is a dynamic role where the successful candidate(s) will receive training and rotate through three different sites. While the successful candidate(s) will have a set schedule, they will also serve as a support resource for sites experiencing staffing challenges. This means their assigned location and shift may change based on operational needs; for example, if a team member takes an unexpected leave of absence, the candidate may be reassigned from a daytime shift at one site to an overnight shift at another to provide necessary coverage ensuring the highest quality of care and support is provided to our residents.

Responsibilities and Duties Include, but are not Limited to:

Case Management and House/Team Responsibilities:

• Assist with task assignments and contextual work explanation under the direction of the Team Lead.
• Aid individuals in financial matters 
• Contribute to the development and implementation of Individual Support Plans (ISP) and Person-Centered Plans (PCP).
• Facilitate medication setup and reconciliation on a weekly basis.
• Conduct home inspections and ensure compliance with regulations and agency policies.

Client Service:

• Foster client-centered approaches to encourage skill development, self-determination, and community inclusion.
• Provide personal care with respect for individual dignity and preferences.
• Implement program activities and goals while networking within the community for inclusion opportunities.

Communication:

• Maintain clear and effective communication with stakeholders through various formats.
• Provide positive feedback to clients, supervisors, and external parties.
• Ensure accurate documentation and reporting in compliance with regulatory requirements.

Teamwork:

• Collaborate respectfully with colleagues and contribute to a positive workplace environment.
• Participate in team meetings and share information for consistency of care.

Required Skills, Education, and Knowledge:

• The candidate(s) must have a relevant 2-year Community College Diploma, related University Degree or equivalent experience in the field.
• Police clearance with vulnerable sector screening.
• A valid G driver's license and insurable by the agency’s insurance company.
• Demonstrated problem-solving skills and ability to respond to individual needs and crisis situations in a calm, productive manner.
• Well-developed organizational and communication skills are essential.
• Experience working with and following Behaviour Support Plans (BSP’s)
• Must be available to work, days, evenings, sleep shifts, weekends, and, must be committed to be available for call in shifts.
