ABOUT THE ROLE
The Outreach Personal Support Worker works in a specialty role, in a close partnership with the Psychogeriatric Resource Consultant (PRC), to provide strategies and support to formal and informal care partners around caring for clients residing in Long Term Care homes (LTCH) that exhibit difficult to support responsive behaviours.

WHAT YOU BRING
Education/Experience:
- Current Personal Support Worker certificate from a recognized college program
- Extensive knowledge and experience working with individuals with various types of dementia, cognitive impairment, delirium, mental health, addictions, and/or neurological conditions and those who experience acute behavioural changes
- Certification in Gentle Persuasive Approaches (GPA) and U-First
Skills/Abilities:
- Demonstrated commitment to people-centered care
- Proven ability to function and contribute as an effective team member and to work independently
- Ability to communicate effectively and concisely, both orally and in writing
- Ability to establish and maintain effective working relationships in the workplace with clients, families, and the general public
- Strong time management skills and attention to detail
- Flexible with the ability to manage changing priorities
- Must be able to perform the essential duties of the job
- Ability to use independent judgment and maintain confidentiality of records, actions, and information
- Must be at an intermediate skill level in word processing, spreadsheet, presentation software
- Must be competent within the meaning of the Occupational Health and Safety Act
- Demonstrated regular attendance at work.
Assets:
- Demonstrated familiarity with the Fixing Long-Term Care Act and regulations is an asset
- Certification relevant to practice setting (i.e. Gerontology, Dementia) is an asset
- The ability to converse in a second language would be an asset
- Experience with quality improvement processes an asset
Conditions of Employment:
- This position shall be conditional upon a satisfactory Police Vulnerable Sector Check (PVSC) where indicated, to ensure the absence of relevant criminal convictions.
- Ability to travel to various work locations as required and provide own transportation
