ABOUT THE ROLE

Reporting to the Manager, Specialized Geriatric Services (SGS), the Psychogeriatric Resource Consultant (PRC) provides consultation, education, and support to healthcare providers and care partners involved in the care of older adults experiencing neurocognitive disorders (such as dementia or delirium), mental health challenges, and responsive behaviors. The PRC works collaboratively with teams across long-term care homes, the Northwest Local Health Integration Network Home and Community Care (formerly the CCAC), hospitals, Ministry of Health–funded community support agencies, and acute and post-acute care settings, as well as with clients, families, and informal caregivers.
