EMPLOYMENT TYPE: Part-Time, Permanent
HOUR(S) - ROTATION(S): 70 hours (bi-weekly) .
NATURE OF POSITION:
Community Participation Supports provides personalized support to adults with diverse abilities. It operates out of the C.D. Hopkins Centre, and some local community locations and is under the supervision of the Community Participation Services Manager. The hours of operation are generally Monday through Friday, and may include some evening or weekend shifts. Training to provide support in 24-hour care sites will be provided, and if necessary staff may be temporarily assigned to provide support as necessary.
KEY RESPONSIBILITIES:
The successful candidate will be cross-trained in all CPS programs, providing flexible support wherever needed each day. This role is ideal for someone who can quickly and seamlessly adapt to different tasks and environments, offering hands-on assistance wherever required. Whether stepping in to cover a specific program or moving between multiple teams daily, this position ensures all areas receive the support they need to operate smoothly.
Case Management and Team Responsibilities:
- Assist with task assignments and contextual work explanation under the direction of the Team Lead.
- Aid individuals in financial matters such as annual budget planning and tax preparation.
- Contribute to the development and implementation of Individual Support Plans (ISP) and Person-Centered Plans (PCP).
- Facilitate medication setup and reconciliation on a weekly basis.
- Conduct home inspections and ensure compliance with regulations and agency policies.
Client Service:
- Foster client-centered approaches to encourage skill development, self-determination, and community inclusion.
- Provide personal care with respect for individual dignity and preferences.
- Implement program activities and goals while networking within the community for inclusion opportunities.

- Maintain clear and effective communication with stakeholders through various formats.
- Provide positive feedback to clients, supervisors, and external parties.
- Ensure accurate documentation and reporting in compliance with regulatory requirements.

Job Requirements:
- Frequent bending, twisting and pushing
- Valid Standard First Aid & Level C CPR
- Non Violent Crisis Intervention (CPI) Training
- The candidate must have received, at minimum, a high school diploma
- A valid ‘G’ driver’s license
- Must be able to obtain a clean 3 Year Drivers Abstract
- Must be reliable, flexible and able to work independently as well as in a team
- Fluency in French is considered a strong asset
